How to avail service

Step 1
Client/Applicant

  • Client /his family comes to office either as walk-in or referred

Frontline Services Unit

  • Intake interview

Step 2
Client/Applicant

  • Attends the initial counselling and comply to the possible action or intervention

Frontline Services Unit

  • Assessment/ initial counseling
  • Termination – for  cases that can be given action at once.

Step 3
Client/Applicant

  • Client remains in the community

Frontline Services Unit

  • For cases needing further action/social welfare intervention, submit initial assessment to CSWD Officer
  • For review and delegation to social worker assigned in the barangay where client lives
  • For home visitation of social worker
  • Evaluation, recommendation, case consultation

Step 4
Client/Applicant

  • reports at the office

  • Implementation of action, intervention
  • Aftercare monitoring
