1. Present a single receipt worth at least P 1,000 from any establishment in these malls, dated from August 25 to September 7, 2024.
2. Fill out the following:
a. Registration/entry form with their full name, age, complete address, contact number, and email address.
b. ‘Family Tree’ entry sheet, detailing the complete names of family members from grandparents to great-grandchildren, with your Senior Citizen ID and a copy of your APO’s birth certificates.
