Bring the Required Documents on July 5, 2024:
1. Original valid ID (the one submitted with the form)
2. Barangay Indigency (for claimant)
3. Valid ID (Xerox front and back)
4. Baptismal or Marriage Certificate indicating Mayor Strike as godfather
