Mayor's Permit

REQUIREMENTS

  1. Health Card / Health Permit
  2. Referral / Recommendation from the Mayor (If not a resident of Bacoor)
  3. Police or NBI Clearance
  4. Barangay Clearance

STEPS IN SECURING A WORKING PERMIT

  1. Get and fill out the application form located at the ground floor or download the Application Form below.
  2. Proceed to the City Health Office Building for the Health Permit Card.
  3. Payments can be made to the City Treasurer’s Office at Windows 4, 5 & 6 (P150.00). Upon payment, proceed to the Management Information system (MIS) Office located at the 3rd Submit the necessary documents (original) for verification.
  4. Encoding data and Capturing of the client. Issuance of the Mayor’s Working Permit
  5. Releasing of the card upon signature from the Mayor’s Office located at the 3rd. Floor.
